Massachusetts has no standalone "license" for assisted living. Assisted Living Residences (ALRs) are certified by the Massachusetts Executive Office of Elder Affairs (EOEA) under M.G.L. Chapter 19D and 651 CMR 12.00, and may offer Level I (standard) or Level II (enhanced/Special Care Unit) certification. Nursing homes are separately licensed by the Massachusetts Department of Public Health (DPH), Division of Health Care Facility Licensure and Certification, under M.G.L. Chapter 111, Section 71. Families can verify a community's certification and inspection history at the Mass.gov DPH Health Care Facility search and the EOEA certified Assisted Living Residence list.

MassHealth's Frail Elder Waiver can cover personal care and supports at home or in some community settings for those who qualify, and Senior Care Options (SCO) — MassHealth's integrated Medicare-Medicaid managed care for 65+ — may also help, but neither generally covers ALR room & board.
